Please bring the following items to your appointment:
- Registration Forms
- Photo ID
- Insurance card(s)
- List of Current Medications
- Sunglasses if available (to protect your eyes after dilation)
- A Referral from your Primary Care Physician if Needed
- Glasses, Contact Lenses, and Prescription or Labels for Contact Lenses
New patient appointments usually take 1 ½ - 2 hours. As part of a new patient exam, your eyes will be dilated. While your pupils are dilated you will be sensitive to light and your vision will be blurred. Dilation usually last several hours, but may last longer. You may want to arrange a ride home.
We participate with many major insurance plans however please note: Our ophthalmologist is considered a specialist. Patients with plans that require a referral from a primary care physician (PCP) in order to see a specialist should contact their PCP to obtain a referral prior to their visit. Co-pays will be collected at the time of service. Payment in full is requested at the time of the visit for patients who are self-pay.
